Medical Cannabis Qualifications

How to Qualify?

You must be at least 18 years old and a resident of the State of Ohio or under 18 with a caregiver to be eligible.

Proof of residency is required in the form of a utility bill, bank statement, etc. If a prospective patient is younger than eighteen years of age, a patient registration submission must be accompanied by a caregiver registration submission, in accordance with rule 3796:7-2-03 of the Administrative Code, before it will be considered complete.

Step 1: See A Physician

Establish and maintain a bona fide relationship with a qualified physician who has a “Certificate to Recommend” from the State Medical Board.
A: An in-person visit with a certified physician is required at least once per year.
B: Certified physicians have access to the Patient Registry and will submit their recommendations for a patient to receive medical marijuana directly to the Patient Registry.
C: The physician will need the patient’s valid Ohio driver's license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport.
D: A registered caregiver may possess and administer medical marijuana to patients with whom the caregiver's registration is associated. The certified physician will need similar identification from a caregiver to establish their profile in the Patient Registry.
E: A certified physician can recommend up to a 90-day supply of medical marijuana with three refills (totaling up to 360-day supply if appropriate for the patient).

A: Physicians who wish to recommend medical marijuana in Ohio must have an active Certificate to Recommend (CTR) from the State Medical Board of Ohio. If your current physician does not have an active CTR, you will need to find a certified physican

Qualifing Medical Conditions

Only patients with one of the following medical conditions may currently participate in Ohio’s medical marijuana program: AIDS, amyotrophic lateral sclerosis, Alzheimer’s disease, cancer, chronic traumatic encephalopathy, Crohn’s disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson’s disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette’s syndrome, traumatic brain injury, and ulcerative colitis.

Step 2: Get Registered

A: Your physician will need to submit the complete patient registration submission on your behalf--- All recommendations must come from, and be submitted by, an Ohio state-licensed and currently practicing physician
B: You must pay the required application fee to the State Board of Pharmacy
C: The complete patient registration submission must be received by the State Board of Pharmacy within ninety calendar days of the date on which the recommendation was made
